As evidence of this Promissory Note, all parties identified herein agree that in installments, as set forth, for value received, the undersigned, [NAME] promises to pay to [NAME], the sum of Two thousand five hundred dollars and fifty-six cents ($2,500.56), together with interest from the date above on the unpaid principal balance due at the rate of FIVE PERCENT (5%) per annum. Principal shall be payable monthly in installments of ONE HUNDRED TWENTY-FIVE DOLLARS ($125.00) beginning on November 03, 2008, and on the 11TH of each month thereafter except that the final installment shall be in the amount of the balance of the principal and accrued interest then remaining unpaid on this Note.

All payments shall be payable in lawful currency of the United States of America. The undersigned agrees to pay all costs of collection including reasonable attorneys' fees.

All parties agree that any portion of this Note may be prepaid at any time or from time to time in whole or in part without penalty, premium or permission. Any partial payment under this Section shall be applied to the installments of the Note in the inverse order of their maturities.

All parties stipulate that this Note is the Note referred to in a certain Agreement between [NAME] and [NAME] dated January 29, 2006, and is subject to all of the terms and provisions of the Agreement.

This Note is secured pursuant to and as more specifically set forth in a certain Security Agreement between [NAME] and [NAME] dated the January 31, 2006, and is subject to all of the terms and provisions of the Security Agreement.
